Individualized Family Service Plan (IFSP)
Parents and other members of the team are partners in developing and implementing the IFSP. This plan documents the family’s strengths and needs related to enhancing the infant’s or toddler’s development. The IFSP lists outcomes for the family and their infant or toddler and describes the resources or services needed to achieve the outcomes.
